The bigger question is regulatory rather than technical: consumer fitness data is generally outside HIPAA, but the moment you share it with a provider or sell into a health plan you are in scope, and that changes your architecture rather than adding a checkbox.
Why is a driver app more expensive than a customer app?

A customer opens the app for two minutes; a driver lives in it for a shift while it streams location in the background, manages battery, survives being killed by the OS, works through dead zones and holds state that must reconcile exactly when signal returns.
